SCAN Training and Attestation
The Centers for Medicare & Medicaid Services (CMS) requires Health Plan employees, contractors and providers who serve Medicare Advantage Special Needs Plan (SNP) members to complete an annual SNP Model of Care (MOC) training.
To fulfill the Model of Care (MOC) training requirement:
1) Click on the 2023 SCAN SNP MOC Training for 2024 SNPs and review the training.

3) Complete and submit the Attestation of SCAN SNP Model of Care Training in the form below to indicate that you have completed the required training. TECQ Foundation compliance department will be notified.
